The procedure for a gastric sleeve.  

With a gastric sleeve from Weight Doctors BARIATRICS®, the majority of the stomach is removed. Only a 2 to 3 cm narrow remnant of the stomach remains, which is referred to as a “sleeve stomach,” or sometimes as a “tube stomach.” Because the majority of the stomach is removed, the tube stomach takes over the function of the stomach.  

The gastric sleeve surgery takes place under general anaesthesia. The procedure is minimally invasive and is performed using a laparoscopic keyhole technique . A few small incisions are made in the abdominal wall through which small surgical tools are inserted. The surgery takes just a little over an hour and is performed via the following steps:    

First. After the surgical tools and the camera have been inserted via the incisions  in the abdomen, the abdominal cavity is filled with a gas (specifically, the Weight Doctors use carbon dioxide) to give the surgeon better access and more space, which increases the visibility of the abdominal organs. 

Second. The surgeon uses a medical stapler along the large, rounded long-side of the stomach, which is sometimes referred to as the “greater curvature”. The stapler has two functions: it removes the lower part of the stomach, while at the same time staples are inserted along the incision to connect the two edges of the wound and close the tube stomach. Thanks to modern medical technology, former time-consuming manual suturing is no longer performed. This technical development increases your safety, rapidly shortens the duration of the surgery, and enormously reduces the risk of complications. 

Third. The separated detached part of the stomach is pulled out of the abdomen through one of the working channels using a  bag made of sterile plastic. Then some dye is admitted into the stomach via a special stomach tube. This dye is used to check the seal of the staple sutures along the staple line.  This is another safety feature at Weight Doctors. This is because if there is no leakage of dye, there is a high probability that the staple suturing is tight enough and no digestive juices can enter the abdominal cavity. As soon as the surgeon has completed this test, the surgery is complete.

    The gastric sleeve is now the most common gastric operation worldwide

    The chances of success with regard to the desired weight loss are very high with sleeve gastrectomy: scientific studies show that, on average, patients succeed in losing between 33 and 83 percent of their excess weight. And this is already the case 6-12 months after the procedure. Initial long-term studies also show that the success in losing weight is much more long-term than with many conservative approaches that can help you lose weight. An earlier technique related to the sleeve gastrectomy provides further guidance regarding the expectations for long-term success of the sleeve gastrectomy. The “gastric sleeve-mill operation”, which is no longer practiced today, involved the preparation of a very similar gastric tube. In contrast to today’s gastric sleeve, however, the remaining parts of the stomach were left in the body. In a study, this approach, which is very similar to gastric sleeve surgery, showed a five-year result of 60 percent weight loss.

    But you also need to be aware of this: Some people regain weight after years of continuous weight loss.     Many of our patients are spoiled for choice when deciding which procedure to opt for: would they prefer the gentlest possible procedure, i.e. a stomach reduction without surgery according to the Weight Doctors SLEEVE®principle, or to go all out and opt for stomach surgery from the Weight Doctors BARIATRICS® program?
    With Weight Doctors SLEEVE® in its sharpest version and in the most modern version based on pose®2 endo-sleeve technology, an endoscope is first inserted into the stomach through the mouth, as in a gastroscopy. Using a special device, tiny anchors and special sutures, the stomach wall is first folded along the entire length of the stomach and then sutured immediately afterwards. This process causes two things to happen in the stomach: 1) The stomach is narrowed, i.e. its curved width is significantly narrower. 2) The stomach is shortened and thus reduced in length. Both together lead to a considerably reduced stomach volume, at its peak only approx. 25-30%. The result is that even small amounts of food are sufficient to achieve a feeling of fullness earlier, to feel it for longer and therefore to eat less.
    The procedure is performed in a single endoscopic session under general anesthesia on an outpatient basis. The procedure is quite short and rarely lasts longer than 60 minutes. The effect of the pose®2 ENDO-SLEEVE technology as part of our Weight Doctor SLEEVE® is permanent. It is important with this approach that our patients adhere to a strict liquid-based diet for the first 6-8 weeks. This is the only way to ensure that the anchoring fuses with the tissue of the stomach wall and forms a secure and robust structure.

    Gastric bypass: the gold standard

    The gastric bypass with Weight Doctors BARIATRICS® causes the greatest possible modification among surgical bariatric procedures and is based on modern laparoscopic keyhole surgery. It is performed in two variants: on the one hand, the so-called Roux-en-Y gastric bypass, which is considered the gold standard of gastric bypass surgeries . Or, on the other hand, a gastric bypass can also be performed based on omega-loop bypass (OLB) principle. The latter alternative is also called a mini gastric bypass, although this variant is by no means “mini” and is just as minimally invasive as gastric surgery using the Roux-en-Y principle. 

    The creation of a gastric bypass is achieved by means of a minimally invasive laparoscopy. Before proceeding, the patient is first placed under general anesthesia. Once asleep the surgeon makes a few small incisions  from the outside of the abdomen, through which small surgical tools are inserted 

    First. A small stomach pouch, is created from the upper part of the stomach using a medical stapling tool. The larger part of the stomach is detached and is no longer directly nor immediately involved in the digestion process 

    Second. The small intestine is separated into two parts. One end of the small intestine is brought up to and connected to the newly created stomach pouch. And, the other end of the small intestine, which is still connected to the larger detached part of the stomach, is reconnected to the intestinal tract. This creates a passage for the digestive juices. This means that the rest of the stomach is returned to its original function and participates in digestion a little later. 

    Therefore, most of the ingested food bypasses the stomach and part of the small intestine (up to 90% of the food), so that both food intake and calorie intake are reduced. Gastric surgery to create a gastric bypass takes about 120 minutes.

    Weight Doctors recommends a gastric sleeve surgery, for individuals with a minimum BMI of 40. However, if necessary or preferred, Weight Doctors® also offer other options for significant and effective weight loss 

    At Weight Doctors we always assess an individual’s health, including weight-related co-morbidities like type 2 diabetes, hypertension (high blood pressure) and cardiovascular disease. While a gastric sleeve surgery has been shown to significantly improve, if not completely reverse, weight-related co-morbidities, it is important to always take these into consideration for us to recommend the most appropriate weight loss treatment for you. However, with greater weight loss goals, and existing weight-related co-morbidities, it is more likely  the Gastric Sleeve will be  suitable for you. 

    The prerequisite, as with any other more or less invasive procedures, is that you have exhausted several attempts and different approaches to lose weight. Including, changes to your diet and daily exercise, however neither have brought about significant results. In addition, the Weight Doctors only treat patients over the age of 18.  

    We tend not to advise people to have a gastric sleeve if their excess weight is mainly due to regular consumption of processed, high-calorie foods and drinks, including sweets and bakery products, highly processed snacks and meals, take-away food, high-calorie drinks (soft drinks, juices and energy drinks) and alcohol. Why? Because high-calorie drinks simply flow through the narrowed, reduced stomach tube rather than being slowed down for digestion. Consuming high-calorie foods and drinks provides no nutritional benefit, only ’empty calories’, so the body absorbs the high amount of calories, which prevents weight loss and can cause weight gain. 

    In these cases, a so-called malabsorptive procedure such as a gastric bypass surgery with Weight Doctors BARIATRICS® is a more sensible approach 

    A gastric sleeve is also not suitable for individuals with gastroesophageal reflux disease (GERD). GERD occurs when the acid in the stomach frequently flows back into the esophagus causing heartburn. Following a gastric sleeve heartburn can increase, therefore individuals with a pre-existing condition of GERD are more suitable for a Weight Doctors Endoscopic SLEEVE® in the extended sleeve version.

    What does gastric sleeve surgery involve?
